IMPORTANT SOFTWARE NOTE TO PROGRAMS: (5.15.2013) As the ARC-PA transitions to the revised accreditation process, all application documents, templates, change forms and reports due to the ARC-PA will be developed in Microsoft Office 2010 and may not be compatible with Office 2007 and earlier. Programs are urged to plan accordingly.
All programs submitting a provisional application for consideration on an ARC-PA agenda of September 2013 and March 2014 should have received a zip file in December 2012 of application materials specific to their review cycle. To avoid confusion, there are no application materials on the web site, except for the guidelines to the feasibility study above. Please contact us if you did not receive the materials.

Feasibility Study: Guidelines for the required feasibility study are available here. Please note deadlines for submission in relation to the scheduled site visit.

This page has been updated and replaces the one in the application materials. Please submit this completed document with your application materials.

Within the application narrative for A1.07, please refer readers to Appendix 1.
